PEDIATRIC CARDIOLOGY Surgical Management of Severe Aortic Outtt,)w Obstruction in Lesions Other Than the Hypoplastic Left Heart Syndrome : Use of a Pulmonary Artery to Aorta Anastomosis
نویسنده
چکیده
Between December 1985 and April 1990, 50 infants with a sanely of congenital cardiac lesions other than the 'sooplastic left heart syndrome underwent surgical relief of aortic outflow obstruction by creation of a pulmonary artery to aorta anastomasis . The patients were grouped anatomically by eentriculnarterial align . ment. Nineteen had normally aligned great arteries (group II ; 25 had transposition of the great arteries, all with a univenlrirular heart of left ventricular morphology (group 1I1 ; and 6 had a double-outlet right ventricle Igroup 111) . :111 patients had either aortic stenosis with atresia, subaorlic stenosis or a restrictive ventricular septa) defect . Sisleen had normal arch anatomy ; 34 had arch anomalies consisting of arch hypoplasia in = 171 . coarctation in = 11), interruption of the arch m = 41 and complex arch anomalies In = 21 . Surgery was performed al a median age of 11) days trance 2 to 1811. Of the 50 infants, 33 surnited . No significant difference in early survival (30 days) was noted among the groups of saving

Hemodynamic assessment after palliative surgery for hypoplastic left heart syndrome.
Ten patients with hypoplastic left heart syndrome underwent cardiac catheterization to assess the effectiveness of palliative surgery designed to prepare them for a modified Fontan procedure. The objectives of palliation were to establish unobstructed systemic blood flow, normalize pulmonary blood flow and pressure, and relieve pulmonary venous obstruction. In the first four patients, systemic ...
متن کاملConsiderations in biventricular repair after the Norwood procedure.
OBJECTIVE The Norwood procedure can be applicable as a first stage palliation in children who can eventually undergo a biventricular repair. Although usual management of these patients is a primary neonatal repair, in selected patients staged approach with a Norwood procedure in the neonatal period followed by a Rastelli procedure in the infancy for conversion to two-ventricle physiology has be...

Hybrid stage I palliation combines cardiothoracic surgery and interventional transcatheter procedures for treatment of hypoplastic left heart syndrome. The approach is an alternative to the Norwood procedure, the traditional first stage of surgical palliation. Hybrid stage I palliation involves placing bilateral branch pulmonary artery bands and a patent ductus arteriosus stent through a median...
متن کاملClinical presentation of coronary arteriovenous fistula according to age and anatomic orientation
Background: Coronary arteriovenous fistulas (CAVFs) are direct connections from one or more coronary arteries to cardiac chambers or a large vessel. They are mostly of congenital origin. The aim of this study was to describe clinical presentation and also delineate the course and management of CAVF. Methods: Clinical data, chest x-rays, echocardiographic and angiographic evaluation of...
متن کاملGrowth characteristics of the aortic arch after the Norwood operation.
OBJECTIVES We sought to characterize the growth of the reconstructed aortic arch after the Norwood operation (NO). BACKGROUND The first stage of surgical palliation of hypoplastic left heart syndrome (HLHS), the NO, includes augmentation of the aortic arch with homograft. Growth characteristics of the reconstructed aortic arch, which is comprised of both native aortic tissue and homograft, ha...
